formal Declaration

When a sports association is applying for financial support for members with disabilites the member needs to sign a formal declaration, confirming that the member is disabled in relation to the activity and may need extra help to be able to participate in the activity.

The formal declaration is stored by the association who has to be able to show it as documentation in case of control by the municipality.

Because you're disabled in regards to the activity, you need to sign a new declaration if you switch sports.

If the member can't sign on their own, they can ask a guardian to help. If this isn't possible, you can contact Folkeoplysning og Tilskud.
